By selecting/checking below, the applicant agrees to pay Quality Polygraph Services, Inc., d.b.a. American International Institute of Polygraph the total stated tuition and fees. The Institute agrees to provide the training in accordance with the provisions of the Institute Catalog. Payment of all moneys due shall be a condition of continuing enrollment. Upon satisfactory completion of all academic and skill requirements, and when all financial obligations to the Institute have been met, the Institute will award a diploma reflecting the student has graduated from the training program. While successful completion of an American Polygraph Association (APA) accredited polygraph-training program is one of the necessary prerequisites for APA membership, it is not the only prerequisite. Successful completion of an APA accredited polygraph program does not guarantee acceptance into the American Polygraph Association. The student and Institute understand that this enrollment agreement, which includes the refund policy in the Institute catalog, may not be amended except in writing signed by both parties.

I HAVE RECEIVED A COPY OF THIS ENROLLMENT AGREEMENT AND A CURRENT SCHOOL CATALOG. I MEET OR EXCEED THE ENTRANCE REQUIREMENTS. I AGREE TO PROVIDE PROOF OF QUALIFICATIONS TO ATTEND THIS TRAINING PROGRAM.
